Is smoking shisha harmful? The honest answer

Yes. Waterpipe smoking is considered harmful to health by major health authorities. The World Health Organization (WHO), the German Cancer Research Center (DKFZ) and that Federal Institute for Risk Assessment (BfR) independently come to the same conclusion: shisha is not harmless and not a “healthier” option for smoking.

📌 The most important thing in one sentence: Shisha smoke contains the same group of substances as cigarette smoke - plus additional carbon monoxide from the coal - and the water does little to change that.

Nevertheless, a flat comparison “one shisha = X cigarettes” falls short. Amount consumed, type of tobacco, charcoal and ventilation make a big difference. Let's look at each of the factors.

What's in shisha smoke?

Even if it's just tobacco heated and not burned (more on this in the guide How does a shisha work?), numerous problematic substances are created. The most important are:

  • Carbon monoxide (CO): Mostly occurs when the coal burns - the main risk that is underestimated.
  • Nicotine: from tobacco, is addictive (amount depends on the variety).
  • Tar & fine dust: pollute the respiratory tract and indoor air.
  • Aldehydes (e.g. formaldehyde, acetaldehyde): formed when glycerin and flavors are heated.
  • Heavy metals (e.g. lead, nickel, arsenic, chromium): come from coal and tobacco.
  • Polycyclic aromatic hydrocarbons (PAHs): partly carcinogenic.

Shisha vs. cigarette – the honest comparison

A shisha session is completely different than a cigarette: it lasts longer, has many more puffs and often takes place indoors. That’s exactly what makes the difference – not “one move”.
